All HMO patients must bring a current referral from their primary care physician.

Please bring all diagnostic tests that have been done relating to your scheduled appointment with you or have them faxed to 703-664-2407 prior to your appointment.

Breast patients must pick up their films and/or CD at the location where they had their mammogram done and bring htem to their scheduled appointment. The films and/or CD will be given back at the end of your visit. (HMO breast patients referred by their GYN Physician must obtain a referral from their Primary Care Physician prior to their scheduled appointment.)
